Note: Requests may require a signature to be processed. In addition, if your premiums are paid on a pre-tax basis through elections in a salary reduction agreement (Section 125 Plan), then changes to those elections may be restricted by federal tax law. Please notify your benefits manager or payroll manager before making any changes to any policies obtained or paid for through a Section 125 Plan.
